All pieces of the application, including transcripts, must be received by ACUNS before the deadline.
Online application
The application is administered through SurveyMonkey, which requires an account (accessible here). Confirmation of account registration will be sent via email. This account will allow you to save and track the progress of your application(s), referee reports and transcripts.
Referee reports
Any application submitted must be accompanied by referee reports. You will identify the referees in your online application. The contact information you provide will be used to generate an automatic invitation via email for them to contribute to your application which will include a link and instructions. It is up to you to confirm that they have received this email. If you are applying to more than one ACUNS Award, your referee is required to complete a separate form for each application. Once the referee submits their report, your ACUNS online account will be updated to reflect its completion.
Transcripts
Digital copies of official post-secondary transcripts will be accepted.
It is highly recommended that you order these well in advance of the application deadline. Send one copy of all required official grade transcripts, in a single package to:
